I state that I am the parent or legal guardian (herein after “the Guardian”) of the minor whose name appears above. I understand that the below terms and conditions apply to said minor and to myself. This document is binding on myself, the minor, and any person with legal standing to sue on behalf of said minor.

i3Detroit makes no representation or claims as to the condition or safety of its space, machines, structure, or surroundings, whether or not owned, leased, operated, or maintained by i3Detroit.

The Guardian understands that all activities at i3Detroit are voluntary and that the minor does not have to participate. It is understood that i3Detroit’s space and the activities held within are potentially dangerous or harmful to the minor’s person or property. Additionally, it is understood that the minor may be exposed to communicable diseases while at i3Detroit. The Guardian voluntarily accepts and assumes any risk of injury or damage to the minor or their property. It is also understood that i3Detroit does not provide any insurance coverage for the minor’s person or property. The Guardian acknowledges that they are responsible for the minor’s safety and the minor’s own health care.

The Guardian understands and agrees that at all times the minor must be accompanied by an adult, designated and approved in writing, to supervise the minor in all activities at i3Detroit. At no time may the minor be left unattended at i3Detroit.

In exchange for allowing the minor to participate in i3Detroit’s activities and having access to its space, the Guardian agrees to release i3Detroit from all liability. In addition, the Guardian agrees to indemnify and hold harmless i3Detroit for any injury to the minor’s person or property.

The Guardian understands that this document is complete unto itself and that no oral representations are binding on i3Detroit.

This Release is binding upon the minor, the minor’s parent(s)/guardian(s), any successors in interest, and/or any person(s) suing on the minor’s behalf.

Adult in charge - as agreed to by the minor’s parent(s)/guardian(s)

The Adult in charge has the duty to supervise the youth and will be responsible for the behavior of the youth while at i3Detroit.
